Below is a list of required documents for this property. Please review the list as you are preparing to submit your application.
- USAFA Installation Animal and Pet Control Policy - Download and review the attached document. By proceeding with your application you acknowledge that you have received and read the document.
- AF Form DD 1746 - Please download, complete, and upload completed form.
- AF Form 4422 - Sex Offender Disclosure and Acknowledgement. Please download, sign and upload
- USAFA Fire Arms Registration - Download, Complete, Sign, and Upload completed form.
- Official Orders - If you do not have your orders yet, please submit to a Leasing Agent upon receipt
- Most Recent end of month LES - Upload Most Recent end of month Leave and Earnings Statement (LES)
- Dependent Verification - Only if dependents are not listed on the orders.

Birth Certificate
Adoption/ Custody Paperwork
- Power of Attorney - Only if service member is deployed or unavailable; must include the ability to start, stop and change allotments through DFAS
- Security Forces Screening Consent Form
- Pet vaccination record and photo - Upload if pets are listed on application

- Security Forces Screening Consent Form - All applicants over the age of 18 are required to have their background check run by Security Forces. This form provides consent for applicants' information to be provided to Air Force Academy Security Forces for the purpose of this background check.
