- Official Orders - If you do not have your orders yet, please submit to a Leasing Agent upon receipt.
- AF Form 4422 - Sex Offender Disclosure and Acknowledgement. Please download, sign and upload.
- AF Form DD 1746 - Please download, complete, and upload completed form.
- Referral Form - Please download to fill out and upload
- Dependent Verification - Only if dependents are not listed on the orders. Acceptable documents: DEERS - Defense Enrollment Eligibility Reporting System, Marriage Certificate, Birth Certificate, and/or Adoption/Custody Paperwork.
- Power of Attorney - Only if service member is deployed or unavailable; must include the ability to start, stop and change allotments through DFAS.
- Most Recent end of month LES - Upload Most Recent end of month Leave and Earnings Statement (LES)
Documents required for All Other Applicants
There will be a $30 application fee per person over the age of 18.
